Sometimes it's useful to be able to view and manipulate medical images such as X-rays, CT or MRI scans on your own PC, laptop or tablet. This is particularly important when preparing teaching files or practising for your radiology exams. Finding a good free DICOM viewer can be tricky, especially as there are so many options out there. We have tested may different applications (so you don't have to) and the following are our best picks. We grouped them according to the operating system used because unfortunately there aren't any free viewers that run on both!
A popular software for radiologists working in the UK is currently a programme called 'Horos'. This is a free open source version of the software used by the Royal College of Radiologists for the viva part of the Final FRCR 2B exam, so obviously it makes sense to use it for teaching as well. This programme is only available on Apple computers, hence why so many radiologists own MacBooks.
DICOM images can be displayed using a program called a ‘DICOM viewer'. An Internet search (e.g. For 'DICOM viewer Windows' or 'DICOM viewer Mac') will locate a variety of DICOM viewers, both free and for sale. When DICOM CDs are created, as a convenience they often include DICOM viewers on the CD, or a direct link. Becoming familiar with Horos will help radiology trainees when preparing for this exam. Horos is easy to use and as a full 64-bit medical image viewer for Mac there is a huge amount of functionality contained within. It is updated regularly. We believe Horos is the best free DICOM viewer for Apple MacOS. The basic steps on Mac: 1. Export the image stack from PACS as JPEG images. If your PACS can not export as JPEG, use Osirix to convert from DICOM to JPEG. Make sure the images are de-identified either with PACS or Osirix. Resize all images to 630 x 630 pixels. Copy and paste the entire set of JPEG images to a single blank slide.
There is a paid version of Horos called 'OsiriX MD', which is produced by Pixmeo, however it is expensive so not ideal for basic teaching purposes, although has great functionality. Pixemo also produce a free demo version called 'OsiriX Lite', however there are major limitations placed on this including pop-ups asking you to upgrade to the paid version, performance restrictions, image viewing restrictions and inability to edit the meta-data attached to DICOM images - for example you can't easily re-order series within a study, which may be important if you are preparing cases for teaching or examinations. It is for these reasons that we do not list OsiriX Lite in our recommendations.

As it is cloud based, you can access it on pretty much any device. The service is restricted to medical professionals and sharing can be done to specific groups e.g. your hospital community. The ability for users to comment and tag images is particularly unique and great for learning and teaching.

The application for uploading/viewing outside CD's is called LifeIMAGE. The application provides a solution for viewing external CDs/DVDs brought in from non-Continuum and outside institutions. LifeIMAGE is a web based application that will act as a repository for external CDs that would allow any clinician to view the prior studies for any patient record. The application provides an easy to use Flash based viewer that displays the imported studies regardless of the embedded viewer or format that is inherent to the CD.
A clinician may drop off a CD at your office/fileroom for one of the following reasons:
1. Verbal Consultation
2. Comparison with current study
3. Consult or full interpretation
Please note: for all 3 reasons the clinician or other staff should be directed to the fileroom so they can fill out the outside submission form and start the process.
# 1 involves only uploading into LifeIMAGE. The physician office sends the CD to the Radiology Department File Room using the Outside Submission form, selects Exam Not sent to PACS: Verbal consultation by Radiologist on LifeIMAGE. Radiology library staff uploads into LifeIMAGE and notifies the Radiologist that the study is ready to review with the clinician.
#2 involves uploading to LifeIMAGE and pushing/nominating to McKesson PACS. The physician office sends the CD to the Radiology Department using the online Outside Submission form, selects Exam sent to PACS: To be compared with a current Continuum exam. Radiology staff uploads into LifeIMAGE, pushes study to McKesson PACS and notifies the Radiologist that the study is ready to compare or it will already be in the patient's study jacket when the new study comes in.

#3 involves uploading to LifeIMAGE and pushing/nominating to McKesson PACS. Hospital policy and HIPPA regulations require that every examination in PACS has a dictated report. Therefore, it is very important that the above process is followed so we can be in compliance with this policy. The ability to upload directly into McKesson will soon be disabled and LifeIMAGE will become our repository for outside CD's.
